Athletics

Greenville Public Schools requires all athletes to have a completed physical form on file, with physician's approval, before a student may try out or participate in a sport.  The MHSAA Physical Form is located by clicking the button to the left.

Physicals completed on or after April 15, 2023 are permitted for the 2023-2024 school year, including the completion of the required MHSAA Health Questionnaire prior to the first day of participation. Students who did not receive a sports physical in the 2022-2023 school year, completed on or after April 15, 2023, must obtain a physical and submit to the athletic office to have on file.

Greenville Public Schools Athletic Department also uses an athletic forms management system called FinalForms for all athletic forms. FinalForms eliminates all the paperwork needed to participate in athletics. Parents/athletes will need to register for an account to get started. You can find directions for creating an account and the website by clicking on the buttons to the left.
